from March 17th to March 24th 328 PENICILLIN IN ENDOCARDITIS STREPTOCOCCUS Viridans Infection Infection of the heart valves with streptococcus viridans occurred in 55 (52%) of 106 patients, 23 had a past history of rheumatic fever ac cording to rising ASLO titers. Surgery in 30 favorable cases consisted of a closed valvotomy in 16 and excision in 14 cases. Their condition was excellent during periods of 1–6 years postoperatively. Penicillin therapy in 30 less favorable cases led to arrest of the infection in 21,; the other 9 died or improved slightly (60 to 90% improvement). One 55yearold man has remained well under continuous penicillin therapy for 5 years. Of 7 patients with operations that failed because of an abscess around the valve, 3 died and 4 improved with secondstage pro cedures. In 2 of 4 valve replacements with a prosthetic valve, the substitute valve became infected, and 3 patients died. The results of chemotherapy with penicillin were found most satisfactory, and surgical intervention was considered when penicillin was not well tolerated or when healing was slow.

Hypertensive Heart Disease

Of 106 patients, 47 had hypertensive heart disease. Twentyeight of these were admitted with chest discomfort. In 28 the heart was moderately or markedly enlarged. Heart failure occurred in 37 cases. Penicillin was administered to 21 of the 47 patients; 11 remained in the hospital for > 3 months. The effect of penicillin was prompt, with improvement of cardiac size, electrocardiographic record, and cardiac function. The dose necessary was 600,000 units daily intramuscularly. The ex ception to the improvement following penicillin was a 65yearold man with cardiac pain, advanced arteriosclerosis, and a cerebral vascular accident; he died of a second cerebral vascular accident 4 months after penicillin was administered. The authors believe that penicillin was not responsible for the accident.

Postoperative Complications

One hundred and one of 106 patients had satisfactory results from a cardiac or thoracic surgical procedure. Secondary operations were performed on 4, and the results were good in 2 and fair in 2. Nine patients had postoperative complications after 66 cardiac operations and 41 thoracic opera tions. The most common complications were: infection in 9 cases, atelectasis in 7, and respiratory insufficiency in 6. Eight patients died postoperatively. Deaths occurred most frequently following operations on the pulmonary artery, esophagus, and trachea. The mortality rate for cardiac operations was 5.4% and 12% for thoracic surgery.

Conclusions

The authors concluded that this study demonstrated that infections following thoracicoabdominal surgery are: (1) common; (2) often severe; (3) due principally to organisms resistant to sulfonamides and penicillin; (4) more often caused by gramnegative organisms than by grampositive organisms; (5) associated with poor hospital hygiene; and (6) best treated, managed, and prevented by cooperative efforts between the surgeon, bacteriologist, and hospital administrator.
